1.Print the Leisure Access Program Application Form.The form is also available:

•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
  o$ z" v1 S4 R, |& K0 u•By calling 780-496-4918 to have one mailed to you.
7 M% ~7 i( l9 L3 X  a•For individuals under custodial care, please also submit the address verification form.
3 `7 _! T8 B$ {0 }2 a•For individuals under government care, please also submit the consent for release of information form.
