How to Apply
1.Print the Leisure Access Program Application Form.The form is also available:
•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.

2.Mail your completed application to:

Leisure Access Program
14th Floor, CN Tower
PO Box 2359
Edmonton AB T5J 2R7

各健身中心都有，北边Clareview,Stadium 健身中心都可以领取。
