community cultural brokers from New Canadian Health Network partner organizations to

Nature of Work: The Social Worker will report to the manager of mental health services. 3 f" n. z. V L: E' V. g
The position builds working relationships with New Canadian Health Network partners’ staff,
p) |) |$ P+ \6 Knewcomers accessing these services and other institutions and service providers. The Social
. T4 Y% P+ Y2 U; r# ]& ?! [Worker will assist in case planning, conferencing and providing support and advocacy as ( h* C! ?' Q0 d5 b
needed. As required, the Social Worker will assist in accessing community supports for
2 i( q2 z0 G7 r* S8 xnewcomers and their families. The Social Worker provides holistic and flexible initial / u% g( W7 P9 F9 U7 |8 _1 e4 i& N) T
assessment of life circumstances, needs, goals and aspirations and adheres to the client-& S) I0 a( R+ B5 s7 B
centered delivery model.

case plan support for clients who have complex needs.

Qualification
8 i3 s% a, a4 d * Z) O: f% S3 M% ~( g6 o
Relevant undergraduate degree or diploma, or equivalent combination of education and / u0 q8 ^ ^: g/ S8 y- h7 b
experience in Social Work or related human service fields.
) R7 G! W& G+ O Extensive knowledge of community resources, including in depth understanding of family : Q. k8 ]1 q) C! |8 f: l* V e5 q
support needs
^9 N& M' g3 r, q `4 A Experience/facility with MS Office computer applications
- L' C' F( T0 s* C Proficiency in both official languages is an asset, as is fluency in other languages 9 i; i0 C3 a. @* I J9 T
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field
