community cultural brokers from New Canadian Health Network partner organizations to : C+ _) o d: j+ s2 C: D

8 h+ f d2 r. }0 E" u! ~, z. x Nature of Work: The Social Worker will report to the manager of mental health services.
# E3 r- W# o+ h3 \The position builds working relationships with New Canadian Health Network partners’ staff, . u9 I: ?) G9 ?" c
newcomers accessing these services and other institutions and service providers. The Social ; L" M! Q3 L; y7 b0 V& H0 x( a
Worker will assist in case planning, conferencing and providing support and advocacy as
, f" U) H1 G, s J* y3 Z, q; U7 f U5 Rneeded. As required, the Social Worker will assist in accessing community supports for 0 U+ M8 @3 I3 G. P3 `. i7 i
newcomers and their families. The Social Worker provides holistic and flexible initial
c& T: o" a" l2 o% E2 C! r& Nassessment of life circumstances, needs, goals and aspirations and adheres to the client-# `: ?! u" D; L
centered delivery model.

Key Accountability

Accesses comprehensive, up-to-date information about services and resources available + g& }1 h3 R$ J1 D, _: u: Z
within the city of Edmonton and New Canadian Health Network partners.
* q+ k4 J( `/ y* g. h! u0 ` Providing holistic information or advocacy services for immigrants and refugees served. 5 |' F" m1 Q) `% D& X( S# b
Developing of social networks and community supports for newcomer families. 5 f3 |9 H4 v3 k" L+ o. }$ I% w9 f
Provide intake, needs assessment, referral, case plans and action plans to support

+ m) z! I# \/ v* ?) _ Work in partnership with settlement workers or cultural brokers to provide supplemental H# j {8 [# \% `
case plan support for clients who have complex needs. 5 ^" U; n& c9 f+ X n/ F) w
Advocate on newcomers’ behalf to programs, institutions and service providers.

% v6 X0 R- z# P9 ^+ \; \ Relevant undergraduate degree or diploma, or equivalent combination of education and
% S1 |# T" t/ m9 j3 n7 S& ~6 Aexperience in Social Work or related human service fields. / b7 q% d* B6 e! j& H
Extensive knowledge of community resources, including in depth understanding of family ; y1 V4 n7 ~) I8 K c& W
support needs
) x8 E9 L( R9 e1 P# \9 [ Experience/facility with MS Office computer applications " {' D6 R9 i- g. |% J
Proficiency in both official languages is an asset, as is fluency in other languages ! V* @% Q$ J& {* c- j/ F
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
