1.Print the Leisure Access Program Application Form.The form is also available:

•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
' M; t# C7 ]$ y4 t) D•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
: W3 t9 p5 }0 J•For individuals under government care, please also submit the consent for release of information form.   

2.Mail your completed application to:
