We encourage you to make every effort to resolve your concern directly with the dentist involved before making a complaint to the Alberta Dental Association and College. Do not delay treatment because you are waiting for the outcome of a complaint.

8 b, O) l- O# d9 V# E- JIf you do make a complaint to the Alberta Dental Association and College about the conduct of a dentist or a quality of care concern, your complaint must:
! b9 |7 u* T% O& U1 ]be in writing;
$ K& ]9 F0 ~0 H
* k! S2 m% L9 D: t/ \: ~be signed by you;
: l2 m6 |# a. i+ w( O8 v* Z- K6 z& c; K1 _
provide the name of the dentist you are complaining about;

; Y( T) g+ Q5 `3 w# J* |include enough detail about the nature of your complaint so the dentist is able to respond to the complaint, such as what happened, when it happened and where it happened;+ H! r* D/ g" L+ Q+ H# D* `% a X
