“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and" I$ S5 w L* C/ B
• there have been no new symptoms or change in symptoms; and/or % f `3 R/ L, _* J/ d1 ~• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. .
