POSITION SUMMARY

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.7 H- D0 f: g4 @8 j

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.5 H3 c& Y" D& m. e [8 H7 x) M) s

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines., N) w0 k! Q w; X0 R
• Ensure the maintenance of accurate records in line with practice policy.

• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

SELECTION REQUIREMENTS:

Qualifications/Experience
- j9 o5 p. }! h. R: ]; J- }• Baccalaureate degree in Nursing preferred
1 [: e* K" [) W) w8 R8 H4 w• Preference given to those with at least 5 years nursing experience in primary care setting.& `1 L& ~7 }) ?. l+ Z3 [
• Current CARNA Registration
/ a% \; l4 N. C9 D8 ]% i7 g• Current CPR – Healthcare Provider- G; ~& Q! H; r6 G
• Must have own vehicle and valid driver’s license" t/ k. d) q4 g/ C4 ]. l2 s3 w
• Knowledge of, and ability to use, electronic medical records5 z5 u# [4 U: x, x
