关于Checklist表(IMM 5612)两个问题:

9 |0 K  x' ~/ ?0 ?8 \1. No. 6 和 No. 11 都提到了fee payment.假如在No.6 提到的Fee payment form (IMM 5620)中选择了信用卡支付Processing Fee,那么No.11还需要提供吗?

2. No. 12的意思是什么? 如果勾选,是否表示用此Checklist作为Cover page,不添加其余材料?

1.查一下你的IMM 5612是不是更新的IMM 5612（03-2009）E.第6项是关于你的护照问题的，不是交费问题。
2.要求的缴费方式是: certified check, bank draft or international money order.
3.IMM 5612（03-2009）E should be included in your package when you are ready to submit your application.
