Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
3 ~) _: A4 E+ t, R9 FPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
# [4 c" k2 |0 E. W6 ]Fees        Ranges from $10 to $35 per week

Remarks:
! R9 Q! G$ ~5 V" w5 ]1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
& K( o. R5 z9 b* w9 E4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
1 B: u5 K$ a1 a6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

" `( Y/ j1 R; w5 {# YWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
