Low Cost Summer Programs Registration Day

Target:        Low income families with children/youth aged 3-17 residing in Edmonton

2 x/ x0 f3 z6 |. ?, x' pPlace        ASSIST Community Services Centre (9653-105A Ave)
+ m1 Y9 J" h0 l% d: D7 GPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
' l+ f, B! O1 g! L1 }) _Fees        Ranges from $10 to $35 per week

1 R7 [7 w; `) Y. r( zRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
. {1 Z; ]% f" Q( ?9 B+ S- L8 W4. Information on Child’s swimming level when registering for swimming classes
8 H( v4 }: a3 @5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

3 s+ K/ u- J; r: w8 p  GFee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
