Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
/ G/ l) r/ I) F2 d: W•For individuals under custodial care, please also submit the address verification form.
; p$ H2 c( ?4 X. S•For individuals under government care, please also submit the consent for release of information form.
