诚意急问大家，领取EI的要求是什么？自身有病导致工作不了可以么？

我有一个亲人有严重的腰痛（据说是风湿和别的一个病），导致不能久坐，久站，不能受风，受寒，不能提重物超过5斤，而且会复发。卧床休息为宜。
4 N! |) N- }$ l& O) C6 H这样的情况，也经过家庭医生鉴定了。可不可以申请EI呢？
7 ~' t; P( g9 U( P% E7 |8 H4 x
" x3 r' D* P7 a, \6 {- e是不是要工作满600小时才可以？ 如果这些小时数满了，但是工作是自己辞职的，在病发前辞职的。6月底辞职的，7月初发病的，突然性的发病。

% p" U3 r! S# Z0 m8 m* u2 Y  P小时数还算不算呢？
* Q. r! q1 r) j) o应该去哪儿申请呢？

只要工作小时够,你的条件应该符合. 不过你是自己主动辞职的,而非因病辞职. 会不会批,得去问政府的人.

病假EI由医生填单，自己寄，100%拿到。单子下载。否则不行。

还有，自己辞职可以，医生单子晚一点也正常。

精打细算

一般如果因为工作长期同一姿势导致的 Soft tissue damage, 可以申请公司的WCB， 同时也可以让专人到公司提供建议如何避免。  
如果WCB 过了， 也可以短暂申请公司的伤残保险中的 STD Short Term Disability， 几个月后还没好就可以转 Long Term Disability.

不过我有2个问题。
您所说的医生指的是家庭医生么？我亲戚她有家庭医生的。然后在这个周一的时候也有大EMS急救车去了大学医院，那边都是有记录的。因为当时非常严重到已经不能走路。

还有就是这个单子在哪儿下载呢？

给预主动辞职充足的理由解释，是可以申请EI并争取得到批准的，详见：
