% n" V3 G! o3 T R; yThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population." |# y1 x4 m) z6 v1 u( y# ^$ {# K. b

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. 9 ^1 W5 X1 s( P; [/ l8 V4 J
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. - T* b/ a6 h7 O2 m; D" @
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.% B' ]3 ^; |8 h! t- a. A3 n) H4 {
• Ensure the maintenance of accurate records in line with practice policy.

SELECTION REQUIREMENTS: J9 t( d7 I4 V' K% H
* c* A+ z( h7 [Qualifications/Experience0 e8 g- ?% a) G4 T C$ E
• Baccalaureate degree in Nursing preferred
/ W% |8 }5 S" N( c1 l2 n# T4 R• Preference given to those with at least 5 years nursing experience in primary care setting.. W! c8 `/ E& T
• Current CARNA Registration4 }; Y; Q+ q/ p1 D0 [9 G( b
• Current CPR – Healthcare Provider% |/ k7 ]7 ]1 P5 M# o
• Must have own vehicle and valid driver’s license+ s1 U0 e& C0 E% [% x1 i; m x) O
• Knowledge of, and ability to use, electronic medical records
