Primary purpose: The Social Worker works in partnership with settlement workers or , H7 c% F, N7 U( @
community cultural brokers from New Canadian Health Network partner organizations to

8 x; ?. C4 {6 y5 b; k Nature of Work: The Social Worker will report to the manager of mental health services. 5 @7 v; R! u ` u( f
The position builds working relationships with New Canadian Health Network partners’ staff,
- @$ h8 w- I; T5 k3 u0 knewcomers accessing these services and other institutions and service providers. The Social
. k$ x2 V, _* H, YWorker will assist in case planning, conferencing and providing support and advocacy as
. o5 F) Q- Y v' q% gneeded. As required, the Social Worker will assist in accessing community supports for
* y7 m. l/ @1 }' P, P: f2 hnewcomers and their families. The Social Worker provides holistic and flexible initial
# r7 q: r6 W8 Xassessment of life circumstances, needs, goals and aspirations and adheres to the client-' y8 O; R' i* ~) f% m2 L2 m
centered delivery model.

Developing of social networks and community supports for newcomer families.

Work in partnership with settlement workers or cultural brokers to provide supplemental 8 c' I0 A5 V! d; p
case plan support for clients who have complex needs.

Relevant undergraduate degree or diploma, or equivalent combination of education and 6 z% X3 C7 Q% Q! ~
experience in Social Work or related human service fields. - k& Y4 b, i+ e Z% K8 _
Extensive knowledge of community resources, including in depth understanding of family ; C4 z, g7 V3 B) f1 ]6 t4 |6 \* C3 S
support needs
# {# k0 X* b3 x! ` Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field
