- V# C0 i( z9 ^4 n Nature of Work: The Social Worker will report to the manager of mental health services. # h: S; ]9 j5 o/ ?* J5 Y5 m8 Q
The position builds working relationships with New Canadian Health Network partners’ staff,
& O% i2 u3 u/ I. o! ~newcomers accessing these services and other institutions and service providers. The Social
0 W0 O5 m, j% R% l$ eWorker will assist in case planning, conferencing and providing support and advocacy as
7 T7 L- C( h% @# j$ Wneeded. As required, the Social Worker will assist in accessing community supports for
4 c2 [' D6 Z6 u% T4 q1 Rnewcomers and their families. The Social Worker provides holistic and flexible initial : }& \1 J& ?+ }+ [
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Understands and adheres to professional ethics related to social work.

) {' I( S4 |* G* O* ]% T7 i Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support - M5 i, g& N) [( p. V
newcomers in settling and fully participating in community.

$ w! d4 |, ~ u7 ^/ Mcase plan support for clients who have complex needs.
; u0 Z" v, T2 a0 B; g' S Advocate on newcomers’ behalf to programs, institutions and service providers. # c& B6 F2 ]$ S! v4 `
Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Relevant undergraduate degree or diploma, or equivalent combination of education and
0 W: G5 I- k! p" O' `" Q' b8 a4 Qexperience in Social Work or related human service fields.
* a* B. n0 j! u3 g. I$ _ Extensive knowledge of community resources, including in depth understanding of family
) X- n% a4 b6 I$ N" y3 N6 vsupport needs 5 d3 O% F) U# H4 g' H- T
Experience/facility with MS Office computer applications 3 L7 D- D3 ^2 Y! C- h
Proficiency in both official languages is an asset, as is fluency in other languages 7 x3 H7 J; C* w! l8 m
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
/ J3 c7 d' b; b8 a; Z! Uinsurance coverage for work

Experience: Minimum of 3 years experience working in a related field 8 a$ r9 E" t. T/ e2 j
