0 N" s: w# p8 V% J2 n! Xcommunity cultural brokers from New Canadian Health Network partner organizations to

3 {* E& Q3 x9 T- d Nature of Work: The Social Worker will report to the manager of mental health services. 5 |8 \7 r$ E- t& x6 N5 p
The position builds working relationships with New Canadian Health Network partners’ staff,
0 O4 f% l% K+ Ynewcomers accessing these services and other institutions and service providers. The Social
% N7 b# R! O# M6 l& n" |) [Worker will assist in case planning, conferencing and providing support and advocacy as : x: g2 h- p) o) w
needed. As required, the Social Worker will assist in accessing community supports for
$ p* F9 u0 {* D4 Cnewcomers and their families. The Social Worker provides holistic and flexible initial * {" N G. A) f
assessment of life circumstances, needs, goals and aspirations and adheres to the client-! L* m% Q: n; Q0 @7 E3 y1 h
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support 8 v1 D% P" l" B+ Y( m
newcomers in settling and fully participating in community.

9 K" X) y% p1 ecase plan support for clients who have complex needs.

Qualification

0 b+ K* S, `* ]; j Relevant undergraduate degree or diploma, or equivalent combination of education and * c& Y* [4 V( C8 I2 |; o; l
experience in Social Work or related human service fields.
3 x" S3 `. w; N$ Y2 R* L* L( A0 I" R Extensive knowledge of community resources, including in depth understanding of family - O8 a% x% t) Y8 i3 H; _

Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
