Primary purpose: The Social Worker works in partnership with settlement workers or # g' O% J0 e7 u& ?/ h6 N
community cultural brokers from New Canadian Health Network partner organizations to % ^3 [" R' P( p7 H8 D2 Q+ h' ^3 V
provide supplemental support to newcomers who have complex needs. 1 v- B7 q: h: T& E, @; \3 K9 y
7 B( ]/ d5 K- |1 [ Nature of Work: The Social Worker will report to the manager of mental health services.
7 K# L8 e! F# g* f4 QThe position builds working relationships with New Canadian Health Network partners’ staff,
5 L5 G0 b$ m& A3 [newcomers accessing these services and other institutions and service providers. The Social
, f+ m! g5 |, N W1 M/ MWorker will assist in case planning, conferencing and providing support and advocacy as 1 S8 f( J1 p5 r: p& p$ b
needed. As required, the Social Worker will assist in accessing community supports for & A. ?8 ~9 D% q. X& g
newcomers and their families. The Social Worker provides holistic and flexible initial $ A- b( Q+ w0 u! v8 s3 L
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support . `/ a( {/ ^2 b3 U' Y

Work in partnership with settlement workers or cultural brokers to provide supplemental 5 q/ ~1 x' {: n& z2 `( _/ x
case plan support for clients who have complex needs.

experience in Social Work or related human service fields.

Experience/facility with MS Office computer applications 2 D, X" ?$ ]8 G& k5 h% S
Proficiency in both official languages is an asset, as is fluency in other languages
9 I- O4 ? U$ ^3 m) D. V) y* R Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 4 U/ l$ S( M( b0 F, k! e' A

Experience: Minimum of 3 years experience working in a related field
