Primary purpose: The Social Worker works in partnership with settlement workers or : W8 @" h H0 y0 x3 T
community cultural brokers from New Canadian Health Network partner organizations to

Nature of Work: The Social Worker will report to the manager of mental health services. 9 t7 I; e4 H3 j
The position builds working relationships with New Canadian Health Network partners’ staff, 8 n) v6 Q: d6 p5 g6 ? t+ P; O$ {
newcomers accessing these services and other institutions and service providers. The Social $ [- I% ^, j! Y" I9 S& k
Worker will assist in case planning, conferencing and providing support and advocacy as
& o& [6 d. S% [. T: E4 ^4 A( ~needed. As required, the Social Worker will assist in accessing community supports for ! p+ W$ d$ T6 z. t) V0 k5 C$ p
newcomers and their families. The Social Worker provides holistic and flexible initial
: }; v4 F5 P s: n+ q$ gassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served. , ~7 B, I% D, G) C7 |' @7 Y

Provide intake, needs assessment, referral, case plans and action plans to support

Extensive knowledge of community resources, including in depth understanding of family

Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 2 d4 O$ c" Q0 Q: g$ n
