Abstract: (2) unique/new employment opportunities in the Transitions Program. As a Case Manager or Community Outreach Specialist you will support clients as they move from the street to living healthier/safer lifestyles. Program services are delivered one-to-one in the areas of life management, crises intervention & management, decision making, community resourcing and employment related services/supports.
