PRIMARY CARE NURSE: d( x  i: `" Y

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.# Y& }5 ~$ B" w) {! o3 F  {( J

( C# h2 q4 l( ?1 y% @% JThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.7 F5 Z5 E4 T) D! R

•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.8 M$ |% S; _2 {) t& \
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.4 g- F) F/ ^$ p% p. l* [
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.0 [" S8 R7 F5 `; t1 ?8 j7 x3 `
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.8 o5 V* D; X2 T0 y9 w# N6 x* T
•        Educator, communicator, advocate, coordination/navigation, supporter# c1 P3 x- Q0 p! k0 ?

Clinical Role

  J  d$ r3 e+ l9 g•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  " \6 U( l, u9 S$ `' m
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
0 ?0 H% d3 ]8 T  W3 v3 }7 |+ i•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines./ ~5 c9 Z: D1 u7 |! }9 y, F' X
•        Ensure the maintenance of accurate records in line with practice policy.- W0 J2 E1 L3 j% C2 g* w5 E
•        To assist in developing and implementing patient self-management programs.
! B1 ^( X( Q% V  j. h•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.: D$ i9 ]! |4 Q7 i- u2 `
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        The primary care nurse should have excellent written and verbal communication skills.

•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.$ x3 f& w+ }' `" {
•        Requires the ability to work independently and prioritize workload.
! W2 X% {  P1 u•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
" c* f! [( U+ o3 e•        To participate in the Network’s education program.
# k$ g2 C+ ?0 P' s6 N* K  e3 Q4 [* t, |& w
SELECTION REQUIREMENTS:' T% S& `# f3 Z+ g

Qualifications/Experience
( @6 U. r8 i4 H2 c' T4 @$ s1 D•        Baccalaureate degree in Nursing preferred
5 o6 e7 a& f5 r6 }, @6 P1 {' _7 o6 ?2 ~•        Preference given to those with at least 5 years nursing experience in primary care setting./ ?7 G2 q1 F" T. @; j7 [
•        Current CARNA Registration
0 b0 M8 L- A6 q8 D7 l& x•        Current CPR – Healthcare Provider
, N% g/ M) I' a•        Must have own vehicle and valid driver’s license5 c4 q' K. x9 B
•        Knowledge of, and ability to use, electronic medical records
