The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.# Y& }5 ~$ B" w) {! o3 F {( J
( C# h2 q4 l( ?1 y% @% JThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.7 F5 Z5 E4 T) D! R

Clinical Role

J d$ r3 e+ l9 g• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. " \6 U( l, u9 S$ `' m
• To develop long-term, trusting relationships with both patients and peers in providing primary care services.

• To assist in developing and implementing patient self-management programs.

• To act as a resource to other multidisciplinary team members, physicians, and community resources.

Qualifications/Experience
( @6 U. r8 i4 H2 c' T4 @$ s1 D• Baccalaureate degree in Nursing preferred
5 o6 e7 a& f5 r6 }, @6 P1 {' _7 o6 ?2 ~• Preference given to those with at least 5 years nursing experience in primary care setting./ ?7 G2 q1 F" T. @; j7 [
• Current CARNA Registration
0 b0 M8 L- A6 q8 D7 l& x• Current CPR – Healthcare Provider
, N% g/ M) I' a• Must have own vehicle and valid driver’s license5 c4 q' K. x9 B
• Knowledge of, and ability to use, electronic medical records
