Nature of Work: The Social Worker will report to the manager of mental health services.
$ y1 H1 P1 E2 |# o& DThe position builds working relationships with New Canadian Health Network partners’ staff,
' J6 B9 d, S3 X' s6 Q, Y% t2 dnewcomers accessing these services and other institutions and service providers. The Social / ^" T! W5 P9 g
Worker will assist in case planning, conferencing and providing support and advocacy as
4 u1 ^* r* B, O) ineeded. As required, the Social Worker will assist in accessing community supports for
8 u8 @$ y1 ^7 G9 Z6 _- {newcomers and their families. The Social Worker provides holistic and flexible initial 2 N, Q' `+ [8 D8 ~
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental
$ _; r3 v2 |. h6 S. [3 O9 C hcase plan support for clients who have complex needs.
+ E- T% |& H; i$ L+ N9 z# i4 L& V Advocate on newcomers’ behalf to programs, institutions and service providers.

Qualification ( x: d4 M5 W) o* w
! T* [# `1 {0 @, s Relevant undergraduate degree or diploma, or equivalent combination of education and
- R$ @& D* x& ~( r- |experience in Social Work or related human service fields.
& n& X$ z7 Z& P" ~# x7 H Extensive knowledge of community resources, including in depth understanding of family
# q' X' t2 a7 H4 Fsupport needs
0 Y( k' C! [7 }+ m' [* } Experience/facility with MS Office computer applications : y3 x F9 M& k6 n- c$ ]* A
Proficiency in both official languages is an asset, as is fluency in other languages

Experience: Minimum of 3 years experience working in a related field
