Nature of Work: The Social Worker will report to the manager of mental health services.
3 E+ C: \$ k0 g4 sThe position builds working relationships with New Canadian Health Network partners’ staff,
1 X0 P+ y$ j3 R) h' v2 ^; X0 Y6 Fnewcomers accessing these services and other institutions and service providers. The Social
$ H e1 f! E2 W- G( {Worker will assist in case planning, conferencing and providing support and advocacy as ' t( X6 R5 B" [: i. }$ U3 k) c( e
needed. As required, the Social Worker will assist in accessing community supports for
6 E1 m" c0 i! ^6 V1 A) onewcomers and their families. The Social Worker provides holistic and flexible initial 9 k& U: O. L' W! \2 c
assessment of life circumstances, needs, goals and aspirations and adheres to the client-0 x p! S/ J$ f. |3 |) j. R( F3 Y, y
centered delivery model.

Relevant undergraduate degree or diploma, or equivalent combination of education and

Extensive knowledge of community resources, including in depth understanding of family 1 \# H( l5 d, q' Z: t
support needs ! `* H0 Y, ]& E5 ~
Experience/facility with MS Office computer applications
. i( w+ Q! { e/ T0 X Proficiency in both official languages is an asset, as is fluency in other languages
$ o2 j$ a+ z1 o& o- A Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate + D! B1 D: m, Q9 B; e1 U2 F1 {

Experience: Minimum of 3 years experience working in a related field
