Nature of Work: The Social Worker will report to the manager of mental health services.
; w9 B6 g. B+ n! o/ `0 \. SThe position builds working relationships with New Canadian Health Network partners’ staff, $ m2 Y: U8 H$ _% Y0 `9 |7 ^
newcomers accessing these services and other institutions and service providers. The Social + `+ t8 c/ h' B. @, u# N
Worker will assist in case planning, conferencing and providing support and advocacy as
2 J D2 j8 z+ a) Y1 I! q. I7 X$ Fneeded. As required, the Social Worker will assist in accessing community supports for
) c, }( K/ x* Z6 P) g1 O8 T: t* bnewcomers and their families. The Social Worker provides holistic and flexible initial
8 S: g) T: [* U/ Z9 w2 }assessment of life circumstances, needs, goals and aspirations and adheres to the client- T6 a& j. D$ K
centered delivery model. 7 R$ s8 R1 H( r6 k- e

Accesses comprehensive, up-to-date information about services and resources available , H; n) A1 _5 I" P) }
within the city of Edmonton and New Canadian Health Network partners. - p7 X. I# u" A
Providing holistic information or advocacy services for immigrants and refugees served.
9 d }( P6 V$ Z Developing of social networks and community supports for newcomer families.
# N% H+ ^: [6 r, ^ Provide intake, needs assessment, referral, case plans and action plans to support ) k0 N( F' ^' N9 i k0 }! t' r& v8 q0 ^
newcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

0 f- X! l- y. V( k% O; S& o7 Y Relevant undergraduate degree or diploma, or equivalent combination of education and
- d4 ? F! \8 a/ Q+ `4 S) T bexperience in Social Work or related human service fields. 9 D( A" ]& C4 H% K$ Q
Extensive knowledge of community resources, including in depth understanding of family $ O) x' ~; U, r0 [3 l& { ^5 p
support needs : j: B* T5 W+ M" s5 O G: i& n
Experience/facility with MS Office computer applications ) ~% G* H# O5 t. |5 J1 H
Proficiency in both official languages is an asset, as is fluency in other languages

Experience: Minimum of 3 years experience working in a related field
