Primary purpose: The Social Worker works in partnership with settlement workers or + u, ^: [" O3 Y9 L8 g5 a
community cultural brokers from New Canadian Health Network partner organizations to . w4 O# q A+ J: h* ^
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
9 V% Y% v& a" [$ ZThe position builds working relationships with New Canadian Health Network partners’ staff, 8 @- d3 K( \1 B9 W5 e3 M" E% l) ]
newcomers accessing these services and other institutions and service providers. The Social
" C) Z" C; r) RWorker will assist in case planning, conferencing and providing support and advocacy as , L$ E9 C1 s1 q8 J
needed. As required, the Social Worker will assist in accessing community supports for
7 g9 P) k! ~4 i! U/ ynewcomers and their families. The Social Worker provides holistic and flexible initial % D2 ~/ k% M9 h g2 i7 y! [. f Q
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support

Extensive knowledge of community resources, including in depth understanding of family - G+ w, w/ |6 F( V; [
support needs . N. S4 b) R w' Z
Experience/facility with MS Office computer applications
