name，email，telephone number，是否UA student或staff， alumni（不是学生需要交membership fee，好像是$12）， onecard number？

# C  D9 @6 c) R5 h. O- \男子三对三排球，报名时间9月10日，需至少3名male player，报名费$20每队。
比赛时间9月20-10月20日，每周一和周三晚上，共四周。

$ a, p( N( {9 j9 N( Y  f* f, w, dRecreational:  These teams are composed of players with no prior competitive volleyball experience.
3 `/ j! C+ M6 gSemi-Competitive:  These teams will have a mixture of skill levels on their team.  It is intended for teams composed of  a mixture of skill levels or with players who play pick-up on a regular basis or who may have played in high school.
6 z3 Y1 H+ N" y1 OCompetitive:  For teams with players who played on their high school squad or who still currently play on a team (ie. City league, etc.)

男女混合排球，报名时间10月5日，需至少男女各3人，报名费$70每队。
6 v/ u; `8 y2 A1 I比赛时间10月20-12月8日，每周一至四某个晚上，报名时需要选周几固定下来。
