Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs. , Z9 I v0 M1 }- b4 n$ X

Nature of Work: The Social Worker will report to the manager of mental health services. 8 ^# D7 g/ k/ G8 }; C8 E
The position builds working relationships with New Canadian Health Network partners’ staff, ; v8 F: ~# x9 ^: `) T2 k3 P/ u3 ~
newcomers accessing these services and other institutions and service providers. The Social
- a( \: [& w" w, s2 ]# h% aWorker will assist in case planning, conferencing and providing support and advocacy as
7 Y* u" B5 p( z& f' {. I& n5 m) ]needed. As required, the Social Worker will assist in accessing community supports for " j. ?& \. Y% M5 R( j* I
newcomers and their families. The Social Worker provides holistic and flexible initial
* d! x; X9 s& uassessment of life circumstances, needs, goals and aspirations and adheres to the client-3 h' o- b$ S8 ]. S' A2 I

Accesses comprehensive, up-to-date information about services and resources available

Provide intake, needs assessment, referral, case plans and action plans to support 4 K9 c" w$ e6 q; L# i
newcomers in settling and fully participating in community.

Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Qualification
! B/ b. @# f- r4 a& U2 x f
- T6 I$ X" e/ ~7 L9 T* d6 x1 Q) s Relevant undergraduate degree or diploma, or equivalent combination of education and
# ?& Q/ O6 _ i* aexperience in Social Work or related human service fields. , n6 {% |& s! V( c) V
Extensive knowledge of community resources, including in depth understanding of family
+ m: D% ?" c) Jsupport needs
% f8 x& O2 [7 _ Experience/facility with MS Office computer applications
+ f6 N! G8 }% A) K Proficiency in both official languages is an asset, as is fluency in other languages 3 X6 h! y! K, k$ o6 V
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
& p7 f# O# q4 o2 X' xinsurance coverage for work $ l5 S, y5 i& o+ D2 h9 a4 s

Experience: Minimum of 3 years experience working in a related field
