Primary purpose: The Social Worker works in partnership with settlement workers or 1 c8 ~+ h! l- H
community cultural brokers from New Canadian Health Network partner organizations to 7 L4 e! o' P) s- u& _% {
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
5 c4 C, m7 V% _: l) U1 }2 aThe position builds working relationships with New Canadian Health Network partners’ staff, , s$ X3 r1 N& y% Y1 C
newcomers accessing these services and other institutions and service providers. The Social 6 j! G! ~" h/ B
Worker will assist in case planning, conferencing and providing support and advocacy as
* f# u4 @: T: w0 w$ fneeded. As required, the Social Worker will assist in accessing community supports for 2 `: m( j: ^- O4 Z
newcomers and their families. The Social Worker provides holistic and flexible initial 6 c; E5 h- B' X" P J
assessment of life circumstances, needs, goals and aspirations and adheres to the client-6 O4 p* H1 d/ K2 J/ O# }- c' b; y
centered delivery model. 8 b- Z# P' r8 K. R1 p

Developing of social networks and community supports for newcomer families.
) X+ m$ Z4 S! h5 D9 x0 K Provide intake, needs assessment, referral, case plans and action plans to support
; I7 E! @. w! H( [1 [newcomers in settling and fully participating in community.

Advocate on newcomers’ behalf to programs, institutions and service providers. # E( ^! \( U" ?2 i

Qualification

, g' t( t4 v7 v& U, f9 ]! g) r Relevant undergraduate degree or diploma, or equivalent combination of education and
# T$ S! J0 D# j% g/ T( Eexperience in Social Work or related human service fields. ; @4 W$ v# ~* U& e- Y2 G
Extensive knowledge of community resources, including in depth understanding of family

Experience/facility with MS Office computer applications
3 y% E& k% Q1 e- L; z Proficiency in both official languages is an asset, as is fluency in other languages * s& ^! p) O" b1 g# e% W$ z
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field
