provide supplemental support to newcomers who have complex needs.

; Y9 H( E. n8 Z( l1 {! T' Z Nature of Work: The Social Worker will report to the manager of mental health services. : k5 q5 N7 s" o" q
The position builds working relationships with New Canadian Health Network partners’ staff, $ k- ^; f5 `/ y2 w# m
newcomers accessing these services and other institutions and service providers. The Social
( z% R! X% Z- U" SWorker will assist in case planning, conferencing and providing support and advocacy as
% u# `! d5 T- s0 Qneeded. As required, the Social Worker will assist in accessing community supports for 9 [) i+ E2 g/ H g3 y* y
newcomers and their families. The Social Worker provides holistic and flexible initial + `5 K$ T) B1 F& f6 }
assessment of life circumstances, needs, goals and aspirations and adheres to the client-. |* p0 V5 r. ~4 W! J k* x
centered delivery model.

Provide intake, needs assessment, referral, case plans and action plans to support

Relevant undergraduate degree or diploma, or equivalent combination of education and
. o5 W- b5 v+ Q, m+ C# s/ mexperience in Social Work or related human service fields. % b4 k0 v7 E1 I, x5 F! b, \
Extensive knowledge of community resources, including in depth understanding of family
5 T1 B7 {5 W: b. ^support needs 1 r0 @& [- @5 i+ c7 r0 t
Experience/facility with MS Office computer applications
3 d: A7 k/ F2 |, |! Y7 r Proficiency in both official languages is an asset, as is fluency in other languages
0 h. {" D3 t% e3 P" q5 W* q# }' D [4 ` Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 9 R, L {7 ~) A' |* v) E
insurance coverage for work

Experience: Minimum of 3 years experience working in a related field
