Low Cost Summer Programs Registration Day

Target:        Low income families with children/youth aged 3-17 residing in Edmonton
& A+ `. j( `, Q8 M$ L% N+ {Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
4 Q/ Z! U! f! J% kFees        Ranges from $10 to $35 per week

; s7 I  j( c+ e5 B5 k1 Q+ E0 K6 aRemarks:
) \4 @$ i$ K( O+ N* u5 Z1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
  n! j( r0 A3 Q8 v+ Y: c2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
8 m3 r1 J6 _, C8 s3 B3. Each child is limited to 1 to 3 programs per organization
  Q' E& Y! i) v: ^4. Information on Child’s swimming level when registering for swimming classes
$ a0 ?( d! a( c4 N! [8 l4 r( ~5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

9 O% O% Z: m/ g4 l5 lFee Reduction Pass Applications

' l' I6 R& M  Z# d8 }7 XWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
