Low Cost Summer Programs Registration Day

Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
, y7 ^1 E  O. Q7 u6 qPlace        ASSIST Community Services Centre (9653-105A Ave)
/ S5 _( G& O* l* S% v+ PPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
9 k4 W; t* F; `1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
# P. |0 [5 V( _, C5 O3 t$ ?2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
5 V& V% u8 @  e( ^6 w( F3. Each child is limited to 1 to 3 programs per organization
6 [  b( F" |$ Y4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
* b/ p1 h9 m9 z6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
